Our Response to Copyright Claims

Upon receiving a proper copyright infringement notice, Oregon PDF Forms will take appropriate action, which may include removing or disabling access to the allegedly infringing material. We will also notify the individual or entity that posted the material, providing them with an opportunity to respond.

Counter Notification

If you have received a notification from Oregon PDF Forms that content you posted has been subject to a copyright claim and you believe this to be in error, you may send a counter notification to our Copyright Agent at dmca@oregonpdfforms.com. Your counter notification should include:

  • Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the federal court for the judicial district in which the address is located, or if your address is outside of the United States, for any judicial district in which Oregon PDF Forms may be found, and that you will accept service of process from the person who provided notification of the alleged infringement.
